Joseph’s Home’s medical respite model fills a service gap between hospital and shelter. Serving people experiencing homelessness who are ready to be discharged from a hospital but too frail to recover on the streets or in shelters. The goal of respite care is to stabilize homeless patients’ medical conditions so they can transition safely to self care.
